This numeral adjective means simply 'four thousand.' It appears primarily in the feeding of the four thousand, where Jesus miraculously feeds a crowd of that size (Matt 15:38; Mark 8:9), and in the disciples' later recollection of that event (Matt 16:10; Mark 8:20). Acts 21:38 preserves a different use, where a Roman commander asks Paul whether he is the Egyptian who led four thousand assassins into the wilderness.
